Privacy – Processing of personal data (GDPR) Agency DKV InfoConseilAssurance

Your personal data, including, where applicable, legal and health-related data, without the involvement of a healthcare professional, to which you expressly consent, is processed by InfoConseilAssurance Sprl Agent lié DKV, whose offices are located at: Tomberg 234, 1200 Brussels, and also by DKV Belgium S.A. (employees of the sales, marketing, health network support, claims, contract management, quality control, legal, logistics, statistics, accounting/litigation, compliance, internal audit, IT and call centre departments), with its registered office at 1000 Brussels, Rue de Loxum 25, in accordance with the law of 8 December 1992 on the protection of privacy with regard to the processing of personal data:

for the purpose of managing contractual relationships
for the purpose of issuing collection notices and verifying invoices
for the purpose of potential litigation
for the purpose of collection from a third party responsible for the claim
for the purpose of preventing abuse and fraud
for the purpose of compiling coded data statistics, including big data
for the purpose of IT testing
for staff training
for service quality control
for DKV Belgium S.A. compliance
for the storage of CCTV recordings for the legal period, if you visit our premises
for commercial prospecting for products and services promoted by InfoConseilAssurance SPRL and DKV Belgium S.A.
By providing your personal data, you give InfoConseilAssurance SPRL and DKV Belgium S.A. your express authorisation to process this data and information for and to the extent necessary for the purposes indicated above.

You expressly agree that the data strictly necessary for each of the purposes listed below may be communicated only to the following third parties:

the FSMA¹ for legal reasons
the BNB² for legal reasons
the IPT³ to which you belong, if applicable, for the payment of international tax
the MUNICH RE group, of which DKV Belgium S.A. is a member, for control and reporting purposes
your mutual insurance company for the purpose of enabling your compensation
one or more insurance companies in the event of co-insurance, assistance and/or recovery of expenses in the event of third-party liability for the occurrence of the loss
BPOST to enable better delivery of our mail
quality control in the event that you have submitted a dispute
your insurance intermediary for statistical purposes using coded data, which it will explain and carry out at your request.
This data will be used solely for the above purposes, unless you subsequently expressly object to its use for commercial prospecting purposes.

By sending a dated and signed written request or email addressed to the data controller (see paragraph below) and providing proof of your identity, you may obtain, free of charge if the volume is reasonable, from InfoConseilAssurance Sprl and DKV Belgium SA written communication of your personal data and, where applicable, the correction of any data that is inaccurate, incomplete or irrelevant.

If, at any time, you wish to exercise your rights with DKV or InfoConseilAssurance, you may send a letter to DKV (Rue de Loxum 25, 1000 Brussels) or an email (privacy@dkv.be), attaching proof of your identity. DKV will make every effort to detect and correct the problem. For InfoConseilAssurance, you can send a letter (Tomberg 234, 1200 Brussels) or an email (privacy@infoconseilassurance.be). If no action has been taken on the request 30 days after it was submitted, it will be considered rejected. The President of the Court of First Instance will hear any request concerning these rights if the request has been rejected by DKV or InfoConseilAssurance, depending on which organisation you contacted. In accordance with Article 77 of the GDPR, you may also contact or lodge a complaint with the Privacy Commission (known as the Data Protection Authority as of 25 May 2018) at the following address: 1000 Brussels, Rue de la Presse, 35 (Tel. + 32 2 274 48 00 – Fax + 32 2 274 48 35 – commission@privacycommission.be) or to the supervisory authority in your country of habitual residence, in order to exercise your rights.
